What is nephrotic syndrome?

A nephrotic syndrome is a group of symptoms that indicate damage to the filters of the kidneys. Nephrotic syndrome characterizes the following conditions:

Albuminuria: Too much protein in the urine
Hypoalbuminemia: Too little albumin in the blood
Hyperlipidemia: Too much fat and cholesterol in the blood
Edema: Swelling in the legs, feet, ankles and around the face or eyes

There are multiple proteins in the blood, but in the case of proteinuria, albumin leaks from the blood into the urine. It is a protein that draws out excess fluid from the body and maintains fluid and electrolyte equilibrium level. This fluid is then passed to the bloodstream where kidneys remove them. But when albumin gets leaked, the body’s capacity to soak fluid gets decreased and cause edema.

Nephrotic syndrome occurs because of the problem in the filters of the kidneys called glomeruli. Glomeruli and tubules work together during the filtration process. When the blood is passed through the filters, glomeruli keep the large molecules of the protein. But if the glomeruli are damaged, then some molecules of protein may skip through them, in such a case, tubule return the protein back into the blood.

During nephrotic syndrome, the healthy kidneys may allow 3 gm of protein to leak into the urine in a 24-hour, which is 20 times more than that filtered by the healthy kidneys.

What are the reasons for nephrotic syndrome?

Primary nephrotic syndrome is caused by diseases that affect the kidneys only, such as membranous nephropathy, focal segmental glomerulosclerosis. To influence the kidneys and their filter is the main target of these diseases. FSGS, which is the most common cause of the nephrotic syndrome, occurs when scar tissues form in the parts of the glomeruli while membranous nephropathy occurs because of harmful deposits of immune molecules on the glomeruli.

Nephrotic syndrome is caused by secondary conditions also. These diseases affect multiple parts of the body, such as diabetes or lupus. More than 50% of cases of nephrotic syndrome are caused by diabetes or inflammation.

The other common causes of nephrotic syndrome include:

• Minimal change disease: In this disease, tissues of the kidneys look normal under a microscope but are not filtering properly.
• Renal vein thrombosis: In RVT, a blood clot blocks a vein that drains blood out of the kidney.
• Over the counter pills: Medications such as non steroidal anti-inflammatory drugs have also been linked to nephrotic syndrome.
